A Chance To Grow (ACTG) is a mission-driven, therapist-led non-profit based in Northeast Minneapolis. Our dedicated team delivers life-changing services in the areas of therapy, education, and care – helping thousands of children with developmental disabilities, families and teen parents, educators, child care providers, and adults with special needs across the Twin Cities and beyond.

Why Join Our Home-Based Services Team?

As part of our Home-Based Services team, you’ll play a key role in supporting individuals and families who receive care through Minnesota’s PCA/CFSS, Respite, and Homemaking (245D) programs. This position combines compassionate client support with meaningful administrative coordination, helping ensure that each person’s services run smoothly and consistently.

Someone who is detail-oriented, organized, and motivated by helping clients maintain the services that allow them to live independently at home. If you value teamwork, clear communication, and making an impact behind the scenes and in the field, we’d love to have you on our team.

Position type: Part-Time, 24 hours per week

Pay range: $20.00-$22.00

Benefits: ACTG offers flexible scheduling, hybrid remote options, PTO and Holiday Pay 

Location: Hybrid Options, In-Office 1 Day Per Week

ACTG is located in a beautiful neighborhood in Northeast Minneapolis close to public transit and freeways, downtown, retail, restaurants, and walking trails. 

Description of responsibilities:
  • Hybrid work environment; in-office during initial training and one day per week after, to serve as the primary liaison between clients, case managers, counties, and the Minnesota Department of Human Services (DHS) for coordination of services. Option to work in office more often if desired.
  • Facilitate the day-to-day operations of clients receiving support through Minnesota’s PCA/CFSS, Respite, and Homemaking (245D) programs through administrative tasks.
  • Verify client eligibility and obtain necessary service authorizations to maintain program compliance
  • Collaborate with billing, HR, nursing staff, and leadership to ensure accurate and timely pre-billing data entry for all billable services.
  • Prepare and submit required paperwork and documentation to ensure continuity of care and timely renewals.
  • Maintain accurate client records, caregiver timecards, and program documentation.
  • Support clients and families in understanding their home-based services, budgets, and care responsibilities.
  • Monitor quality of services, communicate care concerns, and assist in resolving issues.
  • Participate in team meetings and contribute to continuous improvement of Home-Based Services operations.
  • Uphold confidentiality, professionalism, and compliance with HIPAA and DHS standards.
  • Assist clinic departments with front desk coverage and other assigned duties.

Experience: 
  • Preferred previous experience in care coordination, home care, or healthcare administration
  • Preferred experience working with spreadsheets (Google Sheets, Excel, etc.) 
  • Preferred familiarity with insurance verification and service authorization processes 
  • Preferred experience maintaining documentation and using electronic care systems 

Skills:
  • Strong written and verbal communication (Required)
  • Attention to detail and accuracy in documentation (Required)
  • Organizational and time management skills (Required)
  • Ability to work independently and collaboratively within a multidisciplinary team (Required)
  • Proficiency in Google Workspace or similar administrative software (Trainable)
  • Compassionate and professional approach when working with clients and families (Required)
  • Problem-solving and adaptability in a fast-paced care environment (Preferred)
