How do I qualify
​for PCA Services?

To get started, contact your county's intake request line to request an in-home PCA assessment with a county contractor (phone numbers below). If the assessment determines that you (or the client) qualifies for PCA services, the county will determine via the MN Dept. of Human Services score panel, the number of PCA units (hours) to be allocated. You will then select a PCA agency of your choice that you would like to work with.

If you select A Chance To Grow as your PCA agency, one of our nurses will visit your home and begin the intake process. This includes assessment of your needs, new client paperwork, and authorization of release if you are currently working with another PCA company.

Offered
​Programs

Clients will have the option to choose one of two PCA programs offered at A Chance To Grow: Traditional PCA or Choice PCA. With the Traditional program, A Chance To Grow will seek to hire a PCA for you. The Choice program allows you to determine who your PCA will be.
Traditional PCA
  • Serves children 0-18 years
  • Professional staff recruits and trains PCAs​ according to client’s needs
​Choice PCA
  • Serves children and adults
  • Clients or their responsible party are responsible for recruiting and supervising the PCA
  • ACTG acts as the fiscal sponsor

Funding and Approvals
In addition to many waiver programs, Medical Assistance or TEFRA both fund PCA services, A Chance To Grow will also accept clients who are enrolled in prepaid medical plans depending on your insurance provider. Please call us at (612) 706-5511 to learn more about payment options.
